Toomey Syringe In emergency airway management, few things are more important than rapid detection of inappropriate endotracheal (ET) tube placement in the esophagus. A simple but inexpensive device known as the Toomey™ Syringe takes advantage of the structural differences between the trachea and esophagus. The trachea is opened by cartilaginous walls, which remain patent when negative pressure is created by retracting the plunger. This does not obstruct the free flow of air through the ET tube and the Toomey™ syringe because the oesophagus has fibromuscular walls which, under negative pressure, collapse when the plunger is drawn back; there is, therefore no possibility of air entering the Toomey™ syringe through the endotracheal tube.
